We’re proud to celebrate Community Health Network, Inc. as Octobers Member of the Month!

Cayuga Community Health Network (CCHN) is a local nonprofit working to build a healthier Cayuga County. Through community partnerships and direct services, CCHN supports people across the lifespan – from infants to older adults – by coordinating care and connecting individuals and families with the resources they need. CCHN’s main program areas include health and wellness trainings and workshops; perinatal, infant, and child health; Medicaid screening and navigation; harm reduction and case management for individuals impacted by substance use; street outreach for people experiencing homelessness; and rapid rehousing services that help individuals and families experiencing homelessness connect with permanent housing. By meeting people where they are and connecting them with health, housing, and social supports, CCHN works to strengthen the well-being of individuals, families, and communities throughout Cayuga County.
